Job summary

Chapters Health seeks a Per Diem Registered Nurse to provide compassionate palliative and supportive care across hospice assignments. You will assess patient needs and coordinate the Plan of Care with the Interdisciplinary Group, delivering skilled nursing interventions to ease symptoms and support families.

Requirements include an active RN license in the state, BLS, and experience in hospice or hospital settings.

Qualifications

• Current RN license in the state of employment.
• Active BLS certification from AHA or Red Cross.
• Minimum 1 year nursing experience in hospice or hospital setting preferred.
• Experience working with an EMR/EHR system.
• Valid driver’s license and automobile insurance per company policy.
• Reliable transportation to meet visit schedule.
• Ability to communicate effectively in English (verbal and written).
• Flexible scheduling to meet Hospice House needs.

Responsibilities

• Provide and manage direct care to patients and families as part of the Interdisciplinary Group, integrating psychosocial and physical care.
• Coordinate the Plan of Care to reflect evolving patient needs.
• Educate patients and families about disease, symptoms, and safety practices.
