Chapters Health System, Inc. seeks a Per diem RN in Virginia to assess patient needs, coordinate care with the IDG, and provide palliative and supportive nursing support to patients and families.

The role requires an active VA RN license, BLS, and flexibility to meet patient needs. Strong EMR/EHR experience and reliable transportation are preferred. Expect to work across hospice house/hospital settings as needed.

Formación

• Current RN license in VA required.
• Active BLS for healthcare professionals.
• Minimum 1 year hospice or hospital experience preferred.
• Experience with EMR/EHR systems.
• Valid driver's license and reliable transportation.

Responsabilidades

• Assess patient/family needs and coordinate Plan of Care with IDG.
• Provide palliative and supportive care to patient/family.
• Complete clinical admissions and explain services to patients/families.
• Obtain physician orders and initiate Plan of Care.
• Document per Company policy and ensure timely communications.
• Maintain professional representation at assigned facilities.
