Job summary

Christus Hospice and Palliative Care, part of the Optum/LHC Group family, seeks a Per Diem RN Case Manager to deliver the Plan of Care for hospice patients and evaluate care quality. You will coordinate nursing activities and collaborate with the care team in a fast-paced environment.

The role emphasizes timely evaluation, comprehensive patient care, and adherence to hospice policies to ensure high-quality outcomes for patients and families.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ year of clinical hospice or related experience.
• Current driver’s license and access to reliable transportation.
• CPR certification preferred or can be completed within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies the patient/family's physical, psychosocial, and environmental needs and re-assesses as needed, no less than every 15 days.
• Initiates and coordinates the plan of care while maintaining accurate and relevant clinical notes regarding the patient's condition.
• Documents problems, appropriate goals, interventions, and patient/family response to hospice care.
• Collaborates with the patient/family, attending physician and other members of the IDG in providing patient and family care daily.
