Job summary

Elite Hospice of Mena, part of the LHC Group and Optum family, is seeking a Per Diem Registered Nurse Case Manager to deliver the Plan of Care for hospice patients. You will evaluate performance, ensure quality care, and adhere to hospice policies while supporting patient and family needs in a dynamic clinical environment.

The role emphasizes timely eligibility assessments, care coordination with the IDG, and accurate documentation.

Qualifications

• Current unrestricted RN licensure in state of practice.
• 1+ years of clinical experience.
• Current driver’s license with reliable vehicle access or public transit.
• CPR certification preferred within 90 days of hire.

Responsibilities

• Make the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Assess physical, psychosocial, and environmental needs and re-assess at least every 15 days.
• Initiate and coordinate the plan of care with accurate clinical notes.
• Document problems, goals, interventions, and patient/family responses to care.
• Collaborate with patient/family, attending physician and IDG members in daily care.
