Job summary

UnitedHealth Group is seeking a Per Diem Hospice RN Case Manager to deliver the Plan of Care for hospice patients. You will evaluate performance, adhere to hospice policies, and ensure quality patient care while supporting the organization's mission and culture.

The role includes completing initial evaluations within 48 hours of referral, coordinating care with the IDG, and documenting patient responses. A valid RN license and 1+ year of clinical experience are required.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies patient/family physical, psychosocial, and environmental needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care with accurate clinical notes.
