Job summary

UnitedHealth Group, through Heart of Hospice (LHC Group), is seeking a Per Diem Registered Nurse Case Manager to deliver hospice care per the Plan of Care. You will evaluate performance and ensure quality patient care while adhering to hospice policies and objectives.

Core duties include initial evaluation within 48 hours, ongoing needs assessment, and coordinating care with the IDG. DOT compliance and patient safety are emphasized.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle or public transportation.
• LA state requirement allows substitution with LPN hospice experience as noted.

Responsibilities

• Makes initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Identify patient/family needs and re-assess at least every 15 days; plan of care initiated and updated.
• Maintain accurate clinical notes on patient condition and care provided.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with patient/family, attending physician and IDG members for daily care.
