Job summary

UnitedHealth Group is seeking a Per Diem Registered Nurse Case Manager for Christus Hospice and Palliative Care, part of the Optum family. You will manage the Plan of Care for hospice patients and uphold quality standards while supporting families and care teams.

Primary responsibilities include initial nursing evaluation within 48 hours of referral, ongoing reassessments every 15 days, coordinating the plan of care, documenting progress, and collaborating with the patient, family, physician,

Qualifications

• Current unrestricted RN licensure in state of practice.
• 1+ year of clinical experience.
• Current driver's license and access to a dependable vehicle or public transportation.
• CPR certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation within 48 hours of referral.
• Identifies physical, psychosocial, and environmental needs and reassesses every 15 days.
• Initiates and coordinates the plan of care with accurate clinical notes.
• Documents problems, goals, interventions, and patient/family response to care.
• Collaborates with patient/family, attending physician, and IDG daily.
