Job summary

UnitedHealth Group is seeking a Per Diem Registered Nurse Case Manager for Heart of Hospice, part of LHC Group, to deliver the Plan of Care for hospice patients and ensure high-quality care. You will evaluate patients, coordinate planning, and document progress with the interdisciplinary group daily.

Requires RN license, 1+ year clinical experience, and a valid driver’s license with reliable transportation. LA-specific experience may apply. Competitive hourly pay and eligible benefits.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical nursing experience.
• Valid driver's license, vehicle insurance, and access to reliable transportation.

Responsibilities

• Makes initial nursing evaluation for hospice eligibility within 48 hours of referral.
• Identify and reassess patient/family physical, psychosocial needs at least every 15 days.
• Initiate and coordinate plan of care with accurate clinical notes.
• Document problems, goals, interventions, and responses to care.
• Collaborate with patient/family, attending physician, and IDG to provide daily care.
