Job summary

Chapters Health System is seeking a Per Diem RN in Ohio to assess and identify patient and family needs, coordinate the Plan of Care with the IDG, and provide palliative and supportive care. The role includes admissions, physician order management, and collaboration with the interdisciplinary team to ensure compassionate, comprehensive care.

The position requires an active RN license in the state, BLS certification, and at least 1 year of nursing experience, with hospice or hospital exposure

Qualifications

• Current RN license in the state of employment.
• Active BLS certification from AHA or Red Cross.
• Minimum 1 year of nursing experience; hospice or hospital preferred.
• Experience with an EMR/EHR system.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess and coordinate Plan of Care with IDG.
• Provide palliative and supportive care to patients and families.
• Perform clinical admissions and obtain physician orders.
• Supervise care by LPNs and aides; coordinate referrals.
• Communicate changes in patient condition to IDG members.
