Job summary

Crescent Hospice is seeking a Registered Nurse Case Manager (RNCM) for per diem shifts in the York, Chester, and Lancaster County area. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through the journey with compassion and clinical excellence.

Qualifications

• Hold current unencumbered RN license.
• Minimum one year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient with computers and EMR systems.
• CPR certification if required by state.
• Valid driver's license, vehicle insurance, and reliable transportation.

• Collaborative Care Champion: Work with Clinical Director to assess eligibility for hospice services.
• Nurturing Direct Care: As an RN Case Manager, coordinate care while ensuring comfort and quality of life.

• Educator and Advocate: Educate patients and families to create personalized care plans.
• Call of Compassion: Participate in call duties as part of the team.
