Job summary
Sanctuary Hospice is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community. You’ll deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey with compassion and clinical excellence.
Hold current RN license, minimum 1 year med-surg/acute care experience; CPR certification may be required.
Qualifications
- Hold current unencumbered license as a Registered Nurse.
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
- Must be computer proficient with EMR background.
- Current CPR certification if required by state.
- Current driver's license and transportation.
Responsibilities
- Empower Through Engagement: respond to clinical referrals with care and expertise.
- Collaborative Care Champion: assess eligibility and suitability for hospice services with Clinical Director.
- Nurturing Direct Care: deliver nursing care and guide patients toward comfort and quality of life.
- Nursing with Heart: apply nursing skills to ease pain and provide solace.
- Educator and Advocate: educate patients and families to participate in personalized care plans.
- Call of Compassion: participate in call duties to be there when it matters most.
