Job summary

Frontier Hospice is seeking a compassionate Per Diem Hospice RN Case Manager (RNCM) to join our nursing team in Chickasha, OK. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey.

Responsibilities include engaging with the interdisciplinary team, assessing eligibility, and educating families to create personalized care plans. Per Diem schedule aligns with patient needs and Coverage considerations.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
• Collaborate with Clinical Director to assess client eligibility for hospice services and plan care.
• Provide direct nursing care to maintain patient comfort and quality of life.
• Deliver nursing care rooted in established principles to relieve pain and support families.
• Educate patients and families to participate in personalized care plans.
• Share call duties as part of a compassionate team effort.
