Job summary

Frontier Hospice is seeking a Per Diem Hospice Registered Nurse Case Manager (RNCM) to join our compassionate nursing team serving Chickasha, Oklahoma. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey.

As a valued member of our interdisciplinary care team, you’ll help ensure every patient receives The Best Care Possible—care grounded in compassion, clinical excellence, and connection.

Qualifications

• Current unencumbered RN license.
• Minimum of 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient with computers and EMR systems.
• CPR certification if state requires.
• Valid driver's license, vehicle insurance, and reliable transportation.

Responsibilities

• Provide hands-on hospice nursing care and coordinate services.
• Collaborate with Clinical Director to assess client eligibility for services.
• Deliver compassionate care and support to patients and families.
• Educate patients and families to participate in care planning.
• Participate in call coverage to ensure timely hospice support.
