Job summary

Dierksen Hospice is seeking a compassionate Registered Nurse Case Manager (RNCM) for per diem coverage in the Hot Springs area. You will deliver hands-on hospice nursing care, coordinate services, and support patients and families throughout the hospice journey.

Qualifications

Current unencumbered RN license.
Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
Proficient with electronic medical records (EMR).
CPR certification as required by state.
Valid driver's license and reliable transportation.

Responsibilities

Respond to clinical referrals with care and expertise.
Collaborate with Clinical Director to assess client eligibility for hospice services.
Provide direct nursing care to maintain comfort and quality of life.
Deliver nursing with evidence-based principles to ease pain and provide solace.
Educate patients and families to participate in personalized care plans.
Participate in call duties as part of team coverage.
