Job summary

Frontier Hospice is seeking a compassionate Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team delivering end-of-life care in Lawton, Oklahoma. You will provide hands-on nursing, coordinate services, and guide patients and families through their hospice journey.

In this role, you’ll collaborate with an interdisciplinary team, enroll patients in appropriate services, educate families, and participate in call duties while upholding our mission to deliver The Best Care

Qualifications

• Minimum one year of RN experience in medical-surgical/acute care (hospice experience a plus).
• Proficient in typing and EMR systems.

Responsibilities

• Deliver hands-on hospice nursing care and coordinate services.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Educate patients and families to empower participation in care plans.
• Provide compassionate, nurse-led care and comfort to patients.
• Participate in on-call duties as part of the team.
