Job summary

Frontier Hospice is seeking dedicated Per Diem Hospice Nurse Practitioners to deliver compassionate patient care in and around Chickasha, OK. You will assess health histories, diagnose medical conditions, and develop personalized care plans while collaborating with patients, families, and the hospice team to ensure holistic, quality end-of-life care.

Qualifications

• Master's degree in nursing; hospice or palliative care experience preferred.
• Current RN and NP licenses in the US; CPR certification.
• CHPN or related hospice/palliative certification preferred.
• Excellent communication and written skills; proficient with EMR.

Responsibilities

• Provide comprehensive hospice medical care under a collaborating physician.
• Evaluate health histories, diagnose conditions, and develop care plans.
• Collaborate with patients, families, and interdisciplinary team.
• Educate patients and families about hospice care and prognosis.
• Ensure compliance with Medicare/Medicaid/state regulations.
