Job summary

Hospice of New Mexico is seeking a Per Diem Registered Nurse Case Manager (RNCM) to support patients and families with compassionate hospice care in Albuquerque and surrounding areas.

The role emphasizes hands-on nursing, coordination of services, and guidance for patients during their hospice journey. Per diem schedule includes Friday, Saturday, and Sunday shifts with a focus on clinical excellence, compassionate care, and teamwork.

Qualifications

• RN license required.
• 1 year RN experience in med-surg/acute care.
• CPR certification as required by state.
• Valid driver's license and transportation.

Responsibilities

• Respond to clinical referrals with care and expertise.
• Collaborate with Clinical Director to assess eligibility for hospice services.
• Provide direct patient care and coordinate services to support comfort and quality of life.
• Deliver nursing care grounded in proven principles to ease pain and provide solace.
• Educate patients and families to participate in personalized care plans.
• Share in on-call duties to ensure timely responsiveness.
