Job summary

Hospice of New Mexico is seeking a compassionate Registered Nurse Case Manager (RNCM) to join our full-time nursing team in Albuquerque. You will deliver hands-on hospice care, coordinate services, and guide patients and families through their hospice journey.

Ideal candidates hold an active RN license, at least 1 year in medical-surgical/acute care, and are proficient with EMR. A driver’s license and reliable transportation are required; a hospice background is a plus.

Qualifications

• Current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficiency with computer systems and EMR; CPR certification if required by state.
• Valid driver's license and reliable transportation.

Responsibilities

• Deliver hands-on hospice nursing care and coordinate services.
• Collaborate with Clinical Director to assess eligibility and plan care.
• Provide comfort, advocacy and education to patients and families.
• Support team through call duties and care planning; uphold compassionate care.
