Job summary

Hospice of New Mexico is seeking a compassionate Registered Nurse Case Manager (RNCM) for per diem shifts in Albuquerque and surrounding areas. You will be part of an interdisciplinary team delivering exceptional end-of-life care, coordinating services, and guiding patients and families through the hospice journey.

The role emphasizes hands-on nursing care, patient comfort, education for families, and participation in after-hours on-call duties.

Qualifications

• Active RN license in good standing.
• Minimum 1 year RN experience in med-surg/acute care; hospice experience a plus.
• Proficient with computers and EMR systems.

Responsibilities

• Empower Through Engagement: respond to referrals with care and expertise.
• Collaborative Care Champion: assess eligibility for hospice services with Clinical Director.
• Nurturing Direct Care: deliver hands-on hospice nursing and comfort.
• Nursing with Heart: provide skilled nursing rooted in compassion and evidence.
• Educator and Advocate: educate families to build personalized care plans.
• Call of Compassion: participate in after-hours call duties.
