Job summary

Del Corazon Hospice is hiring a compassionate Registered Nurse (RN) to provide hospice visits across Albuquerque and surrounding communities. This full-time role offers meaningful work, strong support from an interdisciplinary team, and opportunities to impact end-of-life care.

You will manage symptoms, administer medications, and educate families while working autonomously in the field with after-hours support as needed.

Qualifications

• Active RN license in New Mexico (required).
• At least 1 year of nursing experience (hospice/home health strongly preferred).
• CPR certification preferred.

Responsibilities

• Case Manage hospice patients along with needed visits in Albuquerque and nearby areas
• Availability to make emergency visits after hours M-TH for assigned patients.
• Conduct patient assessments and manage pain/symptom control
• Administer medications and treatments per physician plan of care
• Educate and support patients, caregivers, and families through the hospice journey
• Collaborate with the hospice interdisciplinary team (IDT) including social work, chaplain, aides, and medical director
• Complete timely and accurate documentation in the electronic medical record
• Serve as an advocate for comfort-focused, patient-centered care
