Job summary

Hospice of New Mexico is seeking a full-time Registered Nurse Case Manager (RNCM) to deliver hands-on hospice nursing care, coordinate services, and guide patients and families through their hospice journey in the Albuquerque area.

You will join our interdisciplinary team, work Monday–Friday 8–5, with a comprehensive benefits package including health/dental/vision insurance, PTO, tuition reimbursement, 401(k) matching, and onboarding.

Qualifications

Must hold a Registered Nurse license in good standing.
Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
Computer proficiency with EMR systems and typing.
CPR certification may be required by state.
Valid driver's license and reliable transportation.

Responsibilities

Respond to clinical referrals with compassionate, expert care.
Collaborate with Clinical Director to assess client eligibility for hospice services.
Provide direct nursing care to support comfort and quality of life.
Deliver nursing care rooted in proven principles to ease pain.
Educate patients and families to create personalized care plans.
Share call duties as part of the care team.
