Job summary

Dierksen Hospice is seeking a compassionate Registered Nurse Case Manager to join our per diem nursing team in Texarkana. You will deliver hands-on hospice care, coordinate services, and guide patients and families through all stages of the hospice journey.

As an RN Case Manager, you will collaborate with the Clinical Director, assess eligibility for services, and educate families to create personalized care plans that emphasize comfort and quality of life.

Qualifications

• Holds current unencumbered RN license.
• Minimum one yearRN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient with computers and EMR systems.
• CPR certification if required by state.
• Valid driver's license, vehicle insurance, and reliable transportation.

Responsibilities

• Respond to clinical referrals with care and expertise.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Provide direct patient care and support to maintain comfort and quality of life.
• Deliver nursing with clinically proven methods and compassionate care.
• Educate patients and families to participate in personalized care plans.
• Share call duties to ensure timely, compassionate coverage.
