Job summary

We are Crescent Hospice, seeking a Per Diem Hospice Registered Nurse Case Manager (RNCM) in Conway, SC. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey within our interdisciplinary team.

Responsibilities include engaging with referrals, assessing client eligibility with the Clinical Director, and educating families to create personalized care plans.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one year experience as an RN in a medical surgical/acute care setting; hospice exp a plus.

Responsibilities

• Empower Through Engagement: Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
• Collaborative Care Champion: Partner with Clinical Director to assess eligibility and suitability of clients for hospice services.
• Nurturing Direct Care: As an RN Case Manager, coordinate care while ensuring patient comfort and quality of life.
• Nursing with Heart: Deliver top-notch care rooted in proven nursing principles to ease pain and provide solace.
• Educator and Advocate: Educate patients and families to participate in personalized care plans.
• Call of Compassion: Share in call duties to be there when it matters most.
