Job summary

Crescent Hospice in Columbia, SC, is hiring a Full Time Hospice RN Case Manager (RNCM) to join our caring nursing team. You will deliver hands-on hospice nursing, coordinate services, and guide patients and families through their hospice journey in the community.

The role reports to the Clinical Director and involves educating families, participating in on-call duties, and providing compassionate care focused on comfort, dignity, and quality of life.

Qualifications

• Active RN license
• Minimum 1 year RN experience in medical-surgical/acute care (Hospice a plus)
• Proficient with EMR
• CPR certification (if required by state)
• Current driver’s license and reliable transportation

Responsibilities

• Respond to clinical referrals with compassionate care.
• Collaborate with Clinical Director to assess patient eligibility and service needs.
• Provide direct, hands-on hospice nursing care.
• Deliver care grounded in evidence-based nursing practice.
• Educate patients and families to participate in personalized care plans.
• Share on-call duties as part of the team.
