Job summary

Embrace Hospice in Conway, SC seeks a Weekday RN Case Manager to provide hospice care at patients' homes and facilities. You will coordinate with a multidisciplinary team, follow physician orders, and implement individualized care plans while traveling locally.

The role requires nursing licensure, CPR, and prior RN experience in clinical or hospice settings. You will work weekdays with on-call responsibilities and travel to patient sites within the service area.

Qualifications

• Education: RN from an accredited school; current license and CPR certification.
• Experience: Two years of RN experience in a clinical setting; hospice preferred.
• License: Current valid State Nursing license; CPR certification required.

Responsibilities

• Administers medications and treatments per agency policy and guidelines.
• Perform accurate assessments and develop a written Plan of Care with the patient.
• Document patient status accurately and timely; communicate with the team.
• Coordinate care with physicians, family, and referring agencies.
• Provide education on care plans, symptom control, and end-of-life options.
• Participate in on-call rotation and visits as scheduled.
