Crescent Hospice seeks a dedicated Registered Nurse Case Manager (RNCM) to join our nursing team in Charleston, SC, covering Charleston, Dorchester, and Berkeley counties. This Full-Time role involves delivering hands-on hospice care, coordinating services, and guiding patients and families through their hospice journey.

The position offers a Monday–Friday schedule (8:00 a.m.–4:30 p.m.) with some on-call responsibilities, a supportive team, and comprehensive benefits.

Qualifications

• Active RN license in good standing with no encumbrances.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice experience a plus.
• Proficient in EMR systems and basic computer typing.

Responsabilités

• Respond to clinical referrals with care and expertise; address inquiries compassionately.
• Collaborate with Clinical Director to assess client eligibility for hospice services.
• Deliver direct nursing care as an RN Case Manager to ensure patient comfort and quality of life.
• Apply nursing skills with proven principles to ease pain and provide solace to patients.
• Educate and advocate for patients and families to create personalized care plans.
• Share in call duties as part of the team.
