Job summary

Casa de la Luz East Valley, part of the LHC Group and Optum family, is seeking a Per Diem Registered Nurse Case Manager to deliver the Plan of Care for hospice patients and uphold quality care.

You will perform initial evaluation within 48 hours, assess needs, coordinate care with the IDG, and maintain accurate notes. A valid RN license and 1+ year clinical experience are required; CPR certification preferred.

Qualifications

• Current RN license in state of practice.
• 1+ years of clinical experience.
• Current driver's license and access to dependable transportation.
• CPR certification preferred or able to obtain within 90 days of hire.

Responsibilities

• Make the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify patient/family needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care with accurate clinical notes.
• Document problems, goals, and responses to care.
• Collaborate with patient/family, attending physician and IDG members daily.
