Job summary

UnitedHealth Group is seeking a Per Diem Registered Nurse Case Manager for Geisinger Hospice-Lewistown, part of LHC Group, within the Optum family. The role focuses on delivering hospice care, assessing patient needs, and coordinating care plans with clinicians and families.

The position requires an unrestricted RN license, 1+ year of clinical experience, and a valid driver's license. A CPR certification is preferred. Compensation includes hourly pay with a comprehensive benefits package.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle.

Responsibilities

• Make the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify and reassess patient/family needs at least every 15 days.
• Initiate and coordinate the plan of care with accurate clinical notes.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with patient/family, attending physician and IDG members daily.
