Hospice of the West is seeking a Per Diem Hospice RN Case Manager (RNCM) to join our dedicated nursing team serving the Casa Grande community. You will deliver hands-on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey.

As part of our interdisciplinary care team, you’ll help ensure patients receive The Best Care Possible — care grounded in compassion, clinical excellence, and human connection.

Qualifications

• Current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice exp a plus.
• Proficient with computers and EMR systems.
• CPR certification required if state requires.
• Valid driver's license and reliable transportation.

Responsabilités

• Respond to clinical referrals with care and expertise.
• Collaborate with Clinical Director to assess eligibility for hospice services.
• Provide direct, compassionate direct patient care and support.
• Deliver nursing with evidence-based practices.
• Educate and advocate for patient and family care plans.
• Participate in on-call duties as needed.
