Casa de la Luz East Valley, part of LHC Group and the Optum family, seeks a Per Diem RN Case Manager to deliver hospice care with a focus on timely evaluations and coordinated plans of care.

You will assess patient needs, document progress, and collaborate with families and the IDG to ensure high-quality care in a supportive environment. This role offers a flexible schedule and competitive hourly pay.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver’s license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.
• Preferred: CPR Certification or ability to complete within 90 days of hire.

Responsabilités

• Make the initial nursing evaluation within 48 hours of referral to determine eligibility for hospice services.
• Identify and reassess patient/family needs at least every 15 days.
• Initiate and coordinate the plan of care with accurate clinical notes.
• Document problems, goals, interventions, and patient/family response to hospice care.
• Collaborate with patient/family, attending physician and IDG to provide daily care.
