Job summary

Hospice of the West seeks a compassionate Registered Nurse Case Manager (RNCM) for PRN/as-needed work in Mesa, AZ. You’ll deliver hands-on hospice nursing, coordinate services, and guide patients and families through the hospice journey as part of an interdisciplinary team.

Responsibilities include responding to referrals, assessing client eligibility with the Clinical Director, and providing direct, comfort‑focused care.

Qualifications

• Active RN license without restrictions
• Minimum 1 year RN experience in medical-surgical/acute care; hospice a plus
• Proficient with EMR and computer skills
• Current CPR certification if required by state
• Valid driver's license and reliable transportation

Responsibilities

• Respond to clinical referrals with care and expertise to support exceptional service
• Collaborate with Clinical Director to assess eligibility for hospice services
• Provide direct nursing care to ensure comfort and quality of life
• Deliver nursing care grounded in established principles and compassion
• Educate patients and families to participate in personalized care plans
• Participate in call duties as part of the team
