Job summary

Hospice of the West is seeking a dedicated Registered Nurse Case Manager (RNCM) for a full-time role serving the Peoria and Surprise areas. You will deliver hands-on hospice nursing, coordinate services, and guide patients and families through their hospice journey with compassion and clinical excellence.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Empower Through Engagement: Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch.
• Collaborative Care Champion: Partner with Clinical Director to assess eligibility for hospice services.
• Nurturing Direct Care: As an RN Case Manager, coordinate care while ensuring patient comfort and quality of life.
• Nursing with Heart: Deliver top-notch care rooted in proven principles and techniques.
• Educator and Advocate: Educate and empower patients and families to participate in care plans.
• Call of Compassion: Share call duties to be there when it matters most.
