Job summary

UnitedHealth Group in Arizona is seeking a Per Diem Registered Nurse Case Manager for hospice services. You will evaluate and implement care plans for each patient, perform timely nursing evaluations, and coordinate with physicians and the IDG to ensure quality, compassionate care.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• CPR Certification or ability to complete within 90 days of hire.

Responsibilities

• Makes the initial nursing evaluation in determining eligibility for hospice services within 48 hours of referral.
• Identifies patient/family needs and reassesses at least every 15 days.
• Initiates and coordinates the plan of care with accurate clinical notes.
• Documents problems, goals, interventions, and patient/family response to care.
• Collaborates with patient/family, attending physician and IDG for daily care.
