Job summary

Crescent Hospice is seeking per diem dedicated Hospice Nurse Practitioners to deliver compassionate patient care in and around the Hartsville area. You will evaluate health histories, diagnose conditions, and develop personalized care plans in close collaboration with patients and families.

Responsibilities include coordinating with the hospice team, educating families, and ensuring compliance with hospice regulations.

Qualifications

• Master's degree with hospice or palliative care experience preferred.
• Current unencumbered RN/NP license in the state of practice.
• Specialist certification (CHPN, Pediatrics, or Geriatrics) preferred.
• Excellent verbal and written communication abilities.
• Proficient computer skills and EMR familiarity.
• CPR certification if required by state.
• Valid driver’s license, vehicle insurance, and reliable transportation.

Responsibilities

• Deliver comprehensive medical care and palliation for terminal illnesses under physician supervision.
• Collaborate with attending physician, hospice medical director, and other practitioners for eligibility and ongoing care.
• Review clinical information and coordinate patient care with the interdisciplinary team.
• Educate patients and families on care goals, preventive care, and psychological/spiritual concerns.
• Advise on initial certification/recertification for hospice care to the Hospice Medical Director.
• Maintain up-to-date knowledge of Medicare/Medicaid, state, and federal hospice regulations.
