Job summary

Frontier Hospice is seeking dedicated Per Diem Hospice Nurse Practitioners to deliver compassionate patient care in and around Chickasha, OK. You will evaluate health histories, diagnose, and develop personalized care plans while collaborating with patients, families, and the hospice team.

Responsibilities include delivering palliate medical care, coordinating with the interdisciplinary team, and educating families.

Qualifications

• Master's degree in nursing required; RN/NP license active in state of practice.
• Strong verbal and written communication skills.
• CPR certification and computer proficiency required.

Responsibilities

• Provide comprehensive hospice medical care and symptom management under physician supervision.
• Collaborate with attending physician and hospice team to plan patient care.
• Coordinate with interdisciplinary team to ensure holistic, patient-centered care.
• Educate patients and families on care plans, goals, and resources.
• Advise on initial certification and recertification eligibility for hospice care.
• Stay compliant with Medicare/Medicaid, state, and federal hospice rules.
