Job summary

Holisticare Hospice is currently hiring Per Diem dedicated Hospice Nurse Practitioners to deliver compassionate patient care in and around the Pleasantville Area.

As a Hospice NP, you will evaluate health histories, diagnose and develop care plans, and collaborate with families and clinicians to ensure holistic care. Per Diem scheduling provides flexibility to attend to patient needs while maintaining work-life balance.

Qualifications

• Master's degree in Nursing (MSN) with hospice/palliative relevance.
• Current unencumbered RN and NP licenses in the practicing state.
• CHPN or similar hospice/palliative certification preferred.
• Excellent verbal and written communication abilities.
• Proficient computer skills and EMR experience.

Responsibilities

• Deliver comprehensive medical care for hospice patients, palliate, and manage terminal illnesses under physician supervision.
• Collaborate with attending physician, hospice medical director, and other practitioners to assess eligibility and ongoing needs.
• Coordinate patient care with the interdisciplinary team to ensure a holistic approach.
• Educate patients and families, address preventive care, psychological, and spiritual concerns.
• Provide input on initial certification and recertification for hospice care.
• Maintain up-to-date knowledge of Medicare/Medicaid and state/federal hospice regulations.
