Job summary

Lutheran Social Ministries of New Jersey seeks a Per Diem Nurse Practitioner to support hospice patients across Gloucester and Burlington Counties. The APN will perform face-to-face assessments, provide palliative care visits, and document in the patient record under Medicare Hospice guidelines.

Under Medical Director supervision, the NP collaborates with the Hospice team to ensure compassionate, compliant care.

Qualifications

• Graduation from a master's-level program for nurse practitioners/clinical nurse specialists.
• Graduate pharmacology course (45+ hours) or equivalent integrated into MSN.
• National certification required.
• Licensed and nationally certified as an Advanced Practice Nurse in New Jersey (NJ Board of Nursing).
• Valid driver's license and reliable transportation.

Responsibilities

• Perform patient medical examinations and palliative care visits for Medicare Hospice benefits.
• Document visits in the clinical record and required forms.
• Provide medical and palliative care under supervision of Hospice Medical Director.
• Communicate effectively with Medical Director, Administrator, and the interdisciplinary team.
