Qualifications

Responsibilities

• Implements palliative care program requirements.
• Completes medical visits with patients and coordinates care with Palliative Care team.
• Conduct face to face encounters for re-certification information to Medical Director.
• Provide clinical findings to treating physicians and supervisers.
• Communicate with patients and families at all levels.
• Coordinate communication with agency personnel.
