Job summary

Frontier Hospice is actively seeking Pe Diem Hospice Nurse Practitioners to deliver exceptional patient care in and around Chickasha, OK. As a Hospice NP, you will assess health histories, develop personalized care plans, and collaborate with patients, families, and other clinicians to ensure holistic care.

The role emphasizes compassionate medical care for terminal illnesses, compliant practice under state Nurse Practice Act, and collaboration with the hospice interdisciplinary team.

Qualifications

• Master's degree with at least 1 year hospice or palliative care experience preferred.
• Current unencumbered RN and Nurse Practitioner licenses in state of practice.
• Hospice/palliative care certifications (CHPN) preferred.
• Excellent verbal and written communication abilities.
• CPR certification (if required by state).
• Valid driver's license, vehicle insurance, and access to dependable transportation.

Responsibilities

• Deliver Compassionate Medical Care: provide comprehensive medical care, palliate, and manage terminal illnesses under physician supervision.
• Collaborate for Holistic Patient Care: consult with attending physician, hospice medical director, and other practitioners for eligibility and ongoing care.
• Coordinate and Oversee Patient Care: review clinical information and coordinate care with interdisciplinary team.
• Educate and Counsel: educate patients and families; address preventive care, psychological, and spiritual concerns.
• Recommend Certification/Recertification: provide input on eligibility for initial certification/recertification.
• Stay Informed and Compliant: maintain knowledge of Medicare/Medicaid, state and federal hospice rules.
