Qualifications

• Master's degree with a minimum of one year of hospice or palliative care experience as an RN or NP preferred.
• Excellent verbal and written communication abilities.
• Must be computer proficient.

Responsibilities

• Provide comprehensive medical care and manage terminal illnesses.
• Consult with healthcare practitioners for patient eligibility.
• Coordinate patient care with the interdisciplinary team.
• Engage in patient and family education.
• Stay informed on regulations for hospice services.
