Job summary

Continuum Hospice in Barnstable, MA is currently hiring Per Diem Hospice Nurse Practitioners to deliver compassionate patient care in and around the Barnstable area.

As a Hospice NP with our team, you will evaluate medical histories, diagnose conditions, develop personalized care plans, and coordinate with families and the care team to ensure holistic, dignified end‑of‑life care. We offer a robust benefits package and a hands‑on onboarding program.

Qualifications

• Master's degree with at least one year hospice or palliative care experience.
• Current unencumbered RN and NP licenses in the state of practice.
• CHPN, Pediatrics or Geriatrics certifications preferred.
• Excellent verbal and written communication; EMR proficiency.
• CPR certification; valid driver’s license and reliable transportation.

Responsibilities

• Deliver comprehensive hospice medical care under physician supervision.
• Collaborate with the hospice interdisciplinary team to assess eligibility and care needs.
• Coordinate patient care and educate patients and families.
• Assist with recertification and comply with Medicare/Medicaid/state/federal rules.
• Maintain up-to-date knowledge of hospice regulations.
