Job summary

Holisticare Hospice is seeking Per Diem Nurse Practitioners to provide compassionate hospice care in the Pleasantville area. You will evaluate health histories, diagnose conditions, and develop personalized care plans in collaboration with patients, families and the hospice team.

As a Hospice NP, you will deliver comprehensive medical care for terminal illnesses under physician oversight, ensuring compliance with state Nurse Practice Act.

Qualifications

• Master's degree with a minimum of one year hospice or palliative care experience preferred.
• Current unencumbered RN and NP licenses in the state of practice.
• Hospice/palliative care certification (CHPN) preferred.
• Excellent verbal and written communication abilities.
• Computer proficiency and EMR experience.
• CPR certification and valid driver's license with reliable transportation.

Responsibilities

• Deliver compassionate medical care for terminal illnesses under physician supervision.
• Collaborate with the hospice team to assess eligibility and ongoing care needs.
• Coordinate patient care with the interdisciplinary team to ensure holistic support.
• Educate and counsel patients and families on care goals and options.
• Provide input on initial certification and recertification for hospice care.
• Maintain up-to-date knowledge of Medicare/Medicaid and state/federal regulations.
