Crescent Hospice in Sumter, SC is actively seeking Per Diem Hospice Nurse Practitioners to deliver compassionate, patient-centered care in the Sumter area. You will assess health histories, develop personalized care plans, and collaborate with families and a multidisciplinary team to support comfort and dignity at end of life.

The role requires a Master's degree in nursing with NP licensure, hospice experience preferred, and the ability to travel locally.

Qualifications

• Master's degree required; hospice or palliative care experience preferred.
• Unencumbered RN and NP licenses in the practice state.

Responsibilities

• Deliver comprehensive hospice medical care and palliation under physician supervision.
• Collaborate with attending physician, hospice medical director, and team for patient eligibility and ongoing care.
• Coordinate patient care with the interdisciplinary team to ensure a holistic approach.
• Educate patients and families on care plans, prevention, and psychosocial needs.
