Optum, part of UnitedHealth Group, seeks a Per diem CNA Hospice Aide to provide compassionate, hands-on end-of-life care for hospice patients at home in Sioux Falls, SD. You will support daily living activities, promote comfort and dignity, and work closely with the hospice care team according to each patient’s plan of care.

The role emphasizes safety, observation, and documentation, with required CNA and CPR certifications, and a valid driver’s license with reliable transportation and liability

Formación

• Current CPR certification.
• Valid driver’s license, reliable transportation and liability insurance.
• Source CNA certification meets state requirements.

Responsabilidades

• Provide personal care services to patients.
• Assist with daily living activities and promote comfort, dignity, and emotional well‑being.
• Follow care plan per written instructions from nurse.
• Observe, report and document patient changes.
• Assist with prescribed exercises as instructed by nurse or therapist.
• Perform simple urine tests as required.
• Assist with meals and homemaking tasks.
