Job summary

Frontier Hospice is seeking a Per Diem Hospice Aide (CNA) to deliver compassionate, in-home end-of-life care. You’ll work with our nursing team to support patients’ physical, emotional, and personal needs with dignity.

Qualifications include a high school diploma or GED, an active Oklahoma CNA license, at least 1 year of CNA experience (hospice a plus), CPR if required, and a valid driver’s license. We offer a comprehensive benefits package, PTO, holidays, tuition reimbursement, and retirement

Qualifications

• High School Diploma or GED required.
• Active unencumbered CNA certification in Oklahoma.
• Minimum 1 year CNA experience in acute or long-term care; hospice experience a plus.
• CPR certification if state-mandated.
• Current driver’s license and access to reliable transportation.

Responsibilities

• Provide personalized care following the plan of care and attend to patient and family requests with compassion.
• Assist with personal care tasks, including bathing, oral hygiene, bedding changes, clothing, and grooming.
• Demonstrate exceptional customer service and dedication to patient satisfaction.
• Support families by sharing knowledge and easing their caregiving responsibilities.
• Maintain accurate documentation in the patient’s home and agency records.
