Job summary

UnitedHealth Group is seeking a Per diem CNA Hospice Aide to provide compassionate, hands-on care for patients at home as part of the hospice team. You will support families, promote comfort and dignity, and follow each patient’s care plan with guidance from a registered nurse.

The role emphasizes personal care, daily living assistance, and collaboration with the hospice team to ensure respectful, individualized care in alignment with care plans.

Qualifications

• Current CNA certification as per state requirements.
• CPR certification current and valid.
• Valid driver's license with reliable transportation and liability insurance.

Responsibilities

• Provides personal care services to patients.
• Assists with activities of daily living and mobility.
• Plans meals, assists with feeding, and performs homemaking tasks as needed.
• Observes and reports changes in patient status to nurse.
• Follows written instructions from the RN for care.
• Assists with prescribed exercises and basic testing as directed.
