Job summary

Optum is seeking a CNA Hospice Aide to provide compassionate, hands-on care to patients at home, supporting families during end-of-life care. You will work closely with the hospice care team to promote comfort, dignity, and emotional well-being in alignment with the plan of care.

Responsibilities include assisting with daily living activities, observing and reporting changes, and ensuring safety and proper technique.

Qualifications

• Current CPR certification is required.
• Valid driver's license and reliable transportation.
• Current state CNA certification (or registration) as per state requirements.

Responsibilities

• Provides personal care services to patients
• Assists with daily living activities to promote comfort and dignity
• Plans and prepares meals; assists with feeding as needed
• Performs homemaking and environmental tasks as assigned
• Observes and reports changes in patient's status to the nurse
• Adheres to plan of care and written nursing instructions
• Assists with prescribed exercises and routine tests as directed by nurse/therapist
• Performs and records simple urine tests as instructed
• Other duties as assigned
