Job summary

Sanctuary Hospice is seeking a Per Diem Hospice Aide (CNA) to provide compassionate, hands-on care to patients at the end of life. You will work with the nursing team to meet physical, emotional, and personal needs with dignity and respect in patients' homes or designated care settings.

Responsibilities include following the care plan, assisting with daily care, delivering excellent customer service, supporting families, and maintaining precise documentation.

Qualifications

• High School Diploma or GED required.
• Current unencumbered CNA certificate in the state.
• Minimum 1 year CNA experience in acute or long-term care; hospice a plus.
• CPR certification if state mandates.
• Valid driver's license and reliable transportation.

Responsibilities

• Provide personalized care following the plan of care to ensure patient comfort and well-being.
• Assist with personal care tasks (bathing, oral hygiene, changing bedding, clothing, shampooing).
• Demonstrate exceptional customer service and patient satisfaction.
• Support families by sharing knowledge and guidance during hospice care.
• Maintain accurate documentation in patient records and agency systems.
