Qualifications

• Must maintain a professional attitude and demeanor and communicate effectively with patients and colleagues.
• One year of direct patient care experience in a health care setting preferred.
• Must have a valid driver’s license and reliable transportation.

Responsibilities

• Provide personal care and light housekeeping to terminally ill patients in home or other settings.
• Work under supervision of an RN and participate in the Hospice IDT.
• Document observations and care provided in a timely manner according to the care plan.
• Assist in delivering compassionate, patient-centered hospice services.
