Job summary

Journey Hospice CHHA per diem role in Atlantic and Cape May Counties provides personal care and light housekeeping to terminally ill patients in home or facility settings, under RN supervision. You will participate in the Hospice IDT and document care per the patient plan.

Requires high school diploma or GED, training/certification as Hospice Aide/Homemaker, valid driver’s license, and ability to work in a multidisciplinary team with compassionate communication and reliability.

Qualifications

• Must maintain a professional attitude and demeanor and communicate effectively with patients, caregivers, colleagues, supervisors, and others.
• Must demonstrate optimism, enthusiasm and willingness to work constructively with other health care team members.
• Minimum age of eighteen and ability to work within a multidisciplinary team.

Responsibilities

• Provide personal care and light housekeeping to terminally ill patients in home, facility, or inpatient settings.
• Observe, document, and report observations and care performed per the plan of care.
• Participate as a member of the Hospice Interdisciplinary Team (IDT).
• Maintain timely documentation of services according to the patient’s care plan.
